Our service provides you with several key components in order to facilitate the benefits you deserve that would otherwise go unpaid and back into the insurance providers' bottom line. After checking out you will be forwarded a record release form to be signed and returned, and a privacy and confidentiality policy form signed by Oral Fitness RX which should be kept in your personal records. Primarily you will receive a detailed listed of your insurance benefit package. You will need to send us the claim, your insurance company's information and any written or radiographic material, that pertain(s) to the procedure(s). A professional letter will be drafted and the claim will be forwarded to the insurance company of record. The payment will be made directly from your insurance company to you, the policy holder. A copy of all of the materials forwarded will be sent to either your home or web address. If there are benefits remaining under your insurance annual maximum, and the procedure is an authorized benefit of your plan, you will get reimbursed for your claim.
